The Indiana CTSI Community Health Partnerships is soliciting proposals for the Trailblazer Award and Trailblazer Planning Grant funding opportunities: 

• The Trailblazer Award is for up to $25,000 and is focused on collaborative, community-engaged research projects focused on topics that have the potential to improve health equity in Indiana with optional focus areas of rural health and/or COVID-19. Applications are due June 1, 2021.
• The Planning Grant is for up to $5,000 and is focused on building new partnerships for a future project. Applications are due August 3, 2021.
